Fill in forms before your appointment
Checked against the app on 24 October 2026
Your practitioner may email you a link to forms to fill in before your first session: questions about you, a consent form you sign by typing your name, and sometimes a short questionnaire. Your answers are sealed on your device so only your practitioner can read them.
Step 1: Open the link in the email. It works on a phone, a tablet or a computer, in any recent browser.
Step 2: Fill in one form at a time and press Next. Answers marked “(optional)” can be left blank: you can talk about anything else in person.
Step 3: On the consent form, read it, type your full name (typing your name signs the form), draw a signature if you like, and tick that you agree.
Step 4: Press Send my forms on the last one. Your answers are sent once, at the end; nothing is kept until then.
- If an answer needs fixing, the page says which one and takes you to it.
- The link works for 30 days. If you have not finished, you get one reminder after a day.
- “These forms cannot be opened” means the link has expired, was cancelled or is incomplete. Use the link in your most recent email, or ask the practice for a new one.
- Questionnaires such as the PHQ-9 and GAD-7 are in English.
